- Its done the same way as the big earring holes that are popular now. You start small and work your way up. Yes, the skin will stretch a little each time until it "grows" used to the size. If skin didn't stretch to fit, fat people would explode.
- They probably made several smaller additions to achieve the size over time. As we all know too well, skin stretches like elastic as we gain weight. So by doing it slowly, the skin was able to adapt. I have a hard time understanding why anyone would want to do that to themselves. It undoubtedly involved many surgeries which are extremely painful from what I hear.

While doctors used to do them for most anyone, they are mainly discontinued due to numerous medical issues and complications. Now, since they grow in size at regular intervals because of the nature of the implant, women with them (usually porn stars) get excess fluid drained regularly. Since they are drained of fluid, they don't usually rip. The skin isn't extra, it is just really tightly formed over the large breasts. The largest breasts in the world belong to Maxi Mounds, a pornographic actress, and implants like this can weigh upwards of 20 pounds.
