1. it may vary from which specialist or hospital that you consult. why not select some hospitals/specialist ,make an appointment and discuss with them since not all of them charge at the same rate..
2. Probably nothing. They have socialized medicine. It is a matter of registration and taking your turn. But there are private practitioners. People go to India and other Asiatic countries for non emergency and planed surgery. Can get 5 star hotel type treatment at a resort type facility.
